Project Overview

GATEKEEPER is a European large-scale pilot focused on smart living environments and healthier independent lives for ageing populations. Within this framework, iwelli4ageing extended the iwelli digital health ecosystem to support older adults at home through connected services, intelligent health data management, and enhanced tools for prevention, diagnosis, and chronic disease management. The project was implemented by Syndesis together with NCSR Demokritos.

Syndesis Contribution

Extended the iwelli digital health ecosystem with services connected to the GATEKEEPER platform
Enhanced the med.iwelli.com and mydata.iwelli.com platforms for management of heterogeneous health data
Contributed to tools for diagnostic support, prevention, and chronic disease management for multimorbidity patients with cardiac conditions
Supported the integration of IoT-enabled health and wellbeing services for home-based care
Supported the development of an open-source Info-button tool to enrich EHR/PHR environments with intelligent search and domain knowledge support
